Job Title: Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at HealthCare Support. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care and working closely with our healthcare professionals to ensure the best possible outcomes for our patients.

Responsibilities:
  • Conduct comprehensive assessments of patients' medical histories, physical health, and environmental and psycho-social status.
  • Develop and implement personalized care plans in coordination with patients' primary care physicians.
  • Provide preventive care to patients in the comfort of their own homes.
  • Document progress notes, prepare member workups, and encounter summaries.
  • Help at-risk patients transition from hospitals and other facilities to their own homes.
Requirements:
  • Must be based in the state of New Jersey.
  • Certified as an Advanced Nurse Practitioner in specialty areas.
  • Hold current unencumbered NJ Licensure as a Registered Nurse.
  • Active DEA and CDS registration.
  • Minimum of two (2) years' experience as an Advanced Registered Nurse Practitioner.
  • Master's degree in advanced clinical practice nursing or graduate of post Master's certificate program for Nurse Practitioner.
  • Current provider card in Basic Life Support (BLS).
  • Willingness to travel to patients' homes and have reliable transportation.
Preferred Qualifications:
  • Experience working with geriatric patients.
  • Home care experience.

Pay Details: $117k - $155k a year + 10% annual bonus.
