About the Role

We are seeking a skilled Hospice Clinical Social Worker to join our team at Resource 1 Homecare Staffing. As a Hospice Clinical Social Worker, you will play a vital role in providing psychosocial support and interventions to patients with life-limiting illnesses and their caregivers.

Key Responsibilities
  • Assessment and Intervention: Engage with patients and caregivers to develop rapport and build trust, assessing their psychosocial and emotional condition, as well as their care environment during regular visits.
  • Care Planning: Formulate individualized plans of care for patients based on assessed needs, strengths, limitations, and goals, ensuring patient and caregiver involvement in the care planning process.
  • Collaboration and Communication: Collaborate with healthcare providers and treatment teams to identify and evaluate patient outcomes, and to evaluate variances in expected behavior and appropriateness of care.
  • Quality and Compliance: Follow social work standards of practice and meet or exceed quality goals to ensure the best possible outcomes for patients and families from admission through bereavement care.
Requirements
  • Education and Licensure: Master of Social Work degree from an accredited educational institution, with a minimum of LGSW licensing required; LCSW certification preferred.
  • Experience: 23 years of practice in the healthcare field, with experience in hospice, end-of-life, or critical care preferred.
  • Documentation and Technology: Familiarity and comfort with documenting in an Electronic Medical Record (EMR) system.
  • Physical Demands: Ability to meet the physical demands of the position, with or without reasonable accommodation.
Work Environment

This is a field-based position, with regular visits to patients' homes in Charles County. A valid driver's license and acceptable driving record are required, as well as reliable transportation.

  • Competitive Compensation: $70,000 - $83,000 per year, with mileage reimbursement at the IRS rate.
