CenterWell Home Health is seeking a highly skilled and experienced Clinical Operations Manager to join our team. As a Clinical Operations Manager, you will be responsible for overseeing the day-to-day operations of our home health services, ensuring that patients receive high-quality care and that our team is equipped to provide exceptional service.
Key Responsibilities:
- Coordinate and oversee all direct care patient services provided by clinical personnel.
- Develop, plan, implement, analyze, and organize clinical operations for a specific location.
- Conduct assessments and reassessments of patients, including updating of care plans and interpreting patient needs.
- Manage the assignment of caregivers and oversee the delivery of care to all patients served by the location.
- Assign appropriate clinicians to cases, as needed, and instruct and guide clinicians to promote more effective performance and delivery of quality home care services.
- Monitor cases to ensure documentation is in compliance with regulatory agencies and requirements of third-party payers.
- Coordinate communication between team members, attending physicians, and caregivers to ensure the appropriateness of care and outcome planning.
- Supervise all clinical employees assigned to a specific location and carry out supervisory responsibilities in accordance with company policies and procedures.
- Participate in sales and marketing initiatives and support home care program initiatives around cardio, ortho, wound care, safe strides, and various therapy initiatives.
Requirements:
- Graduate of an accredited School of Nursing.
- Current state license as a Registered Nurse.
- Proof of current CPR.
- Valid driver's license, auto insurance, and reliable transportation.
- Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.
- Home Health experience, highly preferred.
- Management and people leadership experience, required.
- OASIS experience, highly preferred.
- Homecare Homebase (HCHB) experience, preferred.
- CMS PDGEM billing experience, preferred.
