Family Nurse Practitioner

At Primary Health Care Center of Dade, we are seeking a highly skilled and compassionate Family Nurse Practitioner to join our team of healthcare professionals.
Job Summary:The Family Nurse Practitioner will provide high-quality, patient-centered care to pediatric and adult patients in a primary care clinic setting. This role requires strong clinical skills, excellent communication, and a commitment to delivering exceptional patient care.
Key Responsibilities:- Perform physical examinations and preventative health measures within prescribed guidelines and approved protocols.
- Order, interpret, and evaluate diagnostic tests to identify and assess patient's clinical problems and health care needs.
- Record physical findings, formulate plan and prognosis, based on patient's condition.
- Discuss case, as needed, with Medical Director and/or other health professionals to prepare comprehensive patient care plan.
- Prescribe or recommend drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures.
- Refer patients to specialists for consultation or to specialized health resources for treatment, as needed.
- Participate as instructor/facilitator for in-house employee education.
- Participate as instructor/facilitator in community education and outreach.
- Ensure all credentialing, continuing education requirements for continued licensure are current.
- Adhere to all current Primary Healthcare clinical protocols and family practice standards of care.
- Participate in all required Primary Healthcare Centers, HRSA, OSHA, and Emergency Preparedness and other training.
- Adhere to all HIPAA Rules and Regulations.
- Treat staff, providers, visitors, and patients with dignity and respect.
- Participate and help maintain a Patient-Centered Medical Home environment.

- Advanced nursing degree (nurse practitioner) from an accredited college or university.
- Two years' (minimum) experience in supervised medical practice preferred.
Ability to read, analyze, and interpret general medical information. Ability to effectively present information and respond to questions about patient care.
Mathematical Skills:Strong math skills required.
Reasoning Ability:Ability to apply common sense understanding to carry out instructions given either written or oral. Ability to do problem solving.
Certificates, Licenses, Registrations:Licensed in the State of Georgia. Board Certification as appropriate. CPR Certification.
Physical Demands:Sitting, standing, walking, stooping, and stretching are usually required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Work Environment:The noise level in the work environment is usually low.
