Job Title: Family Nurse Practitioner

We are seeking a highly skilled and dedicated Family Nurse Practitioner to join our team at SWLA Center for Health Services, Inc.

Job Summary:

The Family Nurse Practitioner will work collaboratively with our healthcare team to provide high-quality patient care, including diagnosis, treatment, and consultations. This role requires a strong understanding of nursing principles, medical protocols, and effective communication skills.

Responsibilities:
  • Obtain complete medical histories from patients and families
  • Prescribe medications according to therapeutic goals and SWLA protocols
  • Assess, diagnose, implement treatment, evaluate, and monitor patients' health status
  • Perform complete physical examinations on patients
  • Order routine and diagnostic lab and x-ray studies according to standing orders
  • Diagnose acute and chronic illnesses based on clinical findings, laboratory, and x-ray reports
  • Formulate, initiate, and monitor patient management plans using protocols and/or consultation with a supervising physician
  • Refer patients requiring hospitalization or assessment and management of conditions to supervising physicians or appropriate specialists
  • Render emergency care and treatment, including cardiopulmonary resuscitation, starting IV fluids, medications, and suturing lacerations
  • Consult with physicians and other members of the healthcare team as necessary
  • Maintain all patient records to comply with required standards, review records regularly, and obtain physician counter signatures where necessary
  • Attend all medical staff meetings and participate in Primary Health Services Center Q/A program
  • Participate in departmental or organizational quality/continuous performance improvement activity
  • Attain goals or improvements established for the employee
  • Consult with Chief Health Officer/other physicians on a daily basis to ensure compliance with HRSA, JACHO, and NCQA requirements
Requirements:
  • Graduation from an accredited school of nursing or a nurse practitioner program recognized by the Louisiana State Board of Nursing
  • Hold an advanced or graduate nursing degree or an APRN certification and current with the LSBN requirements for CEUs
  • Hold a current, unencumbered LA license to practice nursing and advanced practice registered nursing
  • Experience in writing policies and procedures
  • Proficient in Microsoft Word and Excel
  • Ability to relate with effectiveness to employees of the Health Center
  • Critical thinking skills and an ability to work through processes in an objective manner
  • Sufficient experience to carry out the duties of this position
  • Knowledge of Health Center Practice management and EMR platforms
