Advanced Practice Professional

We are seeking a highly skilled Advanced Practice Professional to join our team in the Gastroenterology Inflammatory Bowel Disease department. As an Advanced Practice Professional, you will play a critical role in providing comprehensive care to patients with gastroenterology and inflammatory bowel disease.
Responsibilities:- Manage comprehensive and individualized care for gastroenterology patients diagnosed with irritable bowel disease, and their families immediately post disposition and in the ambulatory setting.
- Complete consults and/or pre-procedure/evaluations.
- Complete integrated assessments regarding management and comprehensive medication reviews for disposition of gastroenterology patients diagnosed with inflammatory bowel disease.
- Order additional tests or consults as deemed appropriate, in collaboration with gastroenterology faculty or by following established clinical Guidelines and Protocols.
- Perform and communicate results of diagnostic tests following established clinical guidelines and scope of practice.
- Consult and communicate with clinic and hospital staff regarding abnormal findings in the evaluations of patients.
- Perform succinct comprehensive clinical evaluations and document findings in medical record.
- Facilitate patient disposition by coordinating appropriate services and tests for follow-up appointments in the clinic (as applicable) or location of necessary care arrangements.
- Develop a comprehensive individualized plan of care for gastroenterology patients diagnosed with inflammatory bowel disease patients, which includes teaching, disposition planning and necessary referral to other members of the health care team as necessary.
- Demonstrate expertise in caring for patients across the age continuum of their educational experience and certification.
- Identify unstable or complex gastroenterology/inflammatory bowel disease patient care situations and facilitate intervention if appropriate in the clinic setting.
- Recognize emergency situations and initiate effective emergency care.
- Evaluate gastroenterology/inflammatory bowel disease patients and serve as a coordinator for follow up in the ambulatory setting or longitudinal care facility (as applicable).
- Assist in the collection of outcome measures (as applicable).
- Demonstrate critical thinking and diagnostic reasoning skills in clinical decision making by the identification, evaluation and management of the individual gastroenterology/irritable bowel disease needs of patients/families.
- Demonstrate the ability to utilize equipment in a safe manner.
- Bases decisions made/actions taken on the relevant scientific principles, established standards of care and/or research findings.
- Utilize teaching/learning theory and appropriate teaching strategies in the provision of education to nursing staff, hospital staff, students, patients, families and the community.
- Promote an environment that facilitates learning by responding to appropriate patient/family preferences, priorities, and readiness to learn.
- Update own knowledge base throughout the year.
- Complete all identified clinical competencies.
- Serve as a resource person for staff caring for the gastroenterology/inflammatory bowel disease patient.
- Provide educational programs to professional and paraprofessional personnel in area of expertise.
- Participate in community programs, distribution of literature, as appropriate in area of expertise.
- Document the plan of care, interventions, evaluations, and disposition plans in a clear and concise manner for every patient visit in the patient's medical record following approved hospital/UHA policies and compliance standards for documentation.
- Collaborate with other leaders and staff to effectively achieve patient outcome-based improvements in gastroenterology/inflammatory bowel disease.
- Provide leadership in the development and implantation of changes in the gastroenterology/inflammatory bowel disease practice that positively impact patient outcomes.
- Work collaboratively with medical and nursing staff to facilitate creative problem solving and provide professional support.
- Participate and initiate inter-disciplinary approaches to patient care, program development and education. Serves as a professional role model.
- Promote and practices innovation of the expanded role in the delivery of care to gastroenterology/inflammatory bowel disease patients and families throughout the inpatient hospitalization, and the ambulatory setting and/or longitudinal facility.
- Participate and provide leadership for committees and task forces.
- Document hours and submit billing reports, as appropriate.
- Review and interpret hepatology data from sources such as patient registries, clinical and financial reports, length of stay, access to care, and cost per case reports and recommend/implements changes as necessary.
- Participate in clinical trials and research studies with the Principal Investigator.
- Assist gastroenterology Faculty, Administrators and Directors in the collection of outcome measures.
- Other duties/projects as assigned.
- Graduate of a CCNE accredited nursing program with Master's Degree in Nursing.
- National Certification by one of the recognized APRN Certification Centers.
- Licensure or eligibility for licensure, as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
- American Heart Association BLS Certification required with additional Advanced Life Support training at the discretion of the department.
- Internal Medicine or gastroenterology experience preferred.
- Eligible new graduates will be considered.
- ACNP or FNP certification preferred.
