Job Title: Residential Care Coordinator

Key Responsibilities:

  • Develop and implement individualized care plans that cater to the unique needs and goals of each resident.
  • Collaborate with healthcare professionals, family members, and other stakeholders to ensure seamless care coordination.
  • Maintain accurate records and documentation of resident interactions, progress, and any concerns.

Requirements:

  • High School Diploma or equivalent required; degree in a related field preferred.
  • A valid New York State driver's license with a clean driving record.
  • Experience working with individuals with intellectual and developmental disabilities, preferably in a residential setting.
