Position Overview
Sign-On Bonus: $5,000 for Full-time externals only, pro-rated for Part-time
Role: This position focuses on the management of patients with heart failure in a medical office setting.
Schedule: Full-time, 40 hours per week, Monday to Friday, from 8:30 AM to 5:00 PM.
Key Responsibilities:
- Act as a key resource for patients and clinical teams, ensuring effective care management and support for patients enrolled in the Congestive Heart Failure (CHF) program.
- Engage in pre-visit planning and post-visit follow-up to enhance patient care and compliance.
- Collaborate with healthcare providers to establish quality measures and develop care protocols based on recognized standards.
- Manage a panel of high-acuity chronic care patients, providing education and support for heart failure management.
- Conduct regular follow-ups with patients to monitor adherence to treatment plans and coordinate necessary care services.
- Provide telephonic support for urgent patient needs and maintain communication with recently discharged patients.
- Work with the care team to create individualized treatment plans and assist patients in setting self-management goals.
- Administer medications and document patient interactions accurately.
- Coordinate with case managers and other healthcare professionals to ensure comprehensive patient care.
- Maintain records of patient interactions and manage data related to heart failure care.
Qualifications:
- Minimum of three years of experience in Acute Care nursing or equivalent.
- Associate's Degree in Nursing required; Bachelor's Degree preferred.
Skills and Competencies:
- Proficient in computer skills, including data entry and Microsoft Office applications.
- Ability to perform IV insertions and demonstrate effective communication skills.
Licenses and Certifications:
- Current NYS RN License, along with ACLS and BLS certifications.
Work Environment:
This role involves significant manual skills and may require medium to heavy physical effort.
