Care Manager-Care Management-Full Time-Days-Mount Sinai Queens
Job Summary:
We are seeking a highly skilled Care Manager to join our team at Mount Sinai Queens. The successful candidate will be responsible for coordinating the care of patients across the continuum, ensuring quality, clinical, and cost outcomes.
Responsibilities:
- Chart Review: Review patient charts for appropriateness of admission and continued stays.
- Communication: Communicate with various departments to ensure tests are completed and followed up on.
- Abnormalities/Delays: Follow up on abnormalities and delays, documenting, investigating, and treating as necessary.
- Interdisciplinary Rounds: Facilitate interdisciplinary rounds with house physicians, social services, nursing, and ancillary departments.
- Risk Management: Refer cases to Risk Management when appropriate.
- Managed Care Reviews: Conduct concurrent and retrospective managed care reviews.
- Alternate Level of Care: Identify patient stability for Alternate Level of Care, obtain signatures, and complete forms as necessary.
- Data Entry: Enter ICD-9 codes and data entry codes for Care Management worksheets.
- Discharge Planning: Complete forms necessary for discharge planning, including PRI, M11Q, 485, and others as requested by Social Services.
- Managed Care Denials: Notify attending physicians of managed care denials, facilitating concurrent appeals as necessary.

Compensation:
Mount Sinai Health System provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for this role is $ Hourly. Actual salaries depend on a variety of factors, including experience, education, and hospital need.
