Job Summary

We are seeking a skilled and empathetic Hospice Registered Nurse/RN Case Manager to join our team. As a key member of our interdisciplinary team, you will provide direct patient care services, support patients and their families, and empower caregivers with education and resources.

Responsibilities
  • Provide direct patient care services to hospice patients and supportive care to the patient's family
  • Act as primary liaison between physician, patient/family, and hospice team
  • Manage and collaborate with an interdisciplinary group to provide comfort, peace, and dignity
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs
Requirements
  • RN license in the state of requested employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
  • Additional experience in hospital Staff RN, Med Surg, PACU, Float pool, ER or emergency room, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing is a plus
  • Current driver's license and automobile insurance
  • Current CPR certification
