Position Overview:
We are seeking a

Full-Time Hospice

Patient Care Manager, RN

to oversee operations in our hospice care division.

You will report directly to the Executive Director, Administrator, or Senior Patient Care Manager.

Your responsibilities will encompass the daily management of your designated hospice location, overseeing the clinical components of the hospice program, and ensuring the delivery of high-quality care to patients.

You will manage the site in the absence of senior leadership, supervise clinical and administrative functions, and maintain accurate recordkeeping.
Additionally, you will respond to phone inquiries and referrals in a professional and compassionate manner, participate in the on-call nursing schedule, and provide direct skilled nursing care in patients' homes.

Candidate Profile:

Education and Experience:
A Registered Nurse with at least an Associate's Degree in Nursing from an accredited institution is required.

Unless otherwise specified by state regulations, a Bachelor's Degree in Nursing is preferred.
A minimum of three years of direct patient care experience in a clinical environment is required, along with at least one year of management or supervisory experience.
Experience in hospice or home care for two or more years is essential.
A current nursing license in the state of employment is mandatory.

Licenses, Certifications, and Registrations:
A valid nursing license in the state of employment
CPR Certification
Current automobile insurance and a valid driver's license

Personal Attributes and Skills:
Empathy towards the terminally ill and their families during and after the patient's journey.


You must exhibit adaptability by performing a variety of tasks, often shifting from one to another without losing efficiency or composure.

You should be able to communicate effectively with individuals from diverse socioeconomic backgrounds, influence their perceptions, and make informed decisions based on sound judgment. Displaying evidence of patient advocacy and maintaining professionalism, maturity, cooperation, and tact in interactions with colleagues and all members of the hospice team is crucial.

Specialized Knowledge and Skills:


Understanding of the needs of terminally ill patients and their families, along with a solid grasp of the hospice philosophy; familiarity with the roles of various disciplines providing hospice services; exceptional patient assessment capabilities; strong oral and written communication skills; and comprehensive knowledge of managed care principles and regulatory guidelines (including Medicare and Medicaid) are essential.
