Key Responsibilities

  • Provide personal care based on patient assignments and respond promptly to their requests.
  • Communicate effectively with the Case Manager regarding any tasks that cannot be performed.
  • Utilize positive communication techniques while ensuring confidentiality and respect for patients and their families.
  • Ensure patient safety by using equipment correctly and maintaining a safe environment.
  • Deliver personal care services, including bathing, oral hygiene, and changing bed linens as required.
  • Assist patients with dressing and undressing as needed.
  • Plan and prepare nutritious meals, including shopping for ingredients as assigned.
  • Support patients during meals and assist with feeding when necessary.
  • Monitor and record vital signs, including temperature, pulse, and blood pressure, as per care plans.
  • Observe and care for patients' skin to prevent tissue breakdown.
  • Report any significant changes in patients' conditions to the Case Manager.
  • Assist with mobility and exercise as directed by healthcare professionals.
  • Provide assistance with self-administered medications in accordance with state regulations.
  • Help with light laundry tasks as assigned.
  • Maintain cleanliness and orderliness in patients' living areas.
  • Adhere to documentation standards and organizational policies.
  • Participate in quality improvement initiatives and team activities.

Qualifications

  • Completion of state-required training.
  • A minimum of 75 hours of classroom and supervised practical training.
  • At least six months of experience in providing personal care.
  • Must be at least 18 years old.
  • Ability to read, follow written instructions, and document care provided.
  • Understanding of hospice care philosophy and comfort in providing care to terminally ill patients.
  • Strong references from previous employers.
  • Ability to work independently with minimal supervision.
  • Comfortable with end-of-life issues and providing a calm presence in patients' homes.
  • Current CPR certification is required.
  • Exhibit patience, tact, and good personal hygiene.
  • Strong oral and written communication skills.
  • Good organizational abilities.
