We are seeking a compassionate and skilled Registered Nurse Case Manager to join our team at Interim HealthCare. As a Registered Nurse Case Manager, you will play a vital role in delivering high-quality patient care and support to our clients in Belmont, Harrison, and Jefferson counties in Ohio.
Key Responsibilities:- Conduct comprehensive patient assessments and develop personalized care plans
- Oversee the implementation and ongoing assessment of patient care plans
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
- Provide education and support to patients and families on proper home health care procedures
- Work to prevent readmissions by promoting preventative care and ensuring continuity of care
- Current unrestricted license to practice as a Registered Nurse (RN) in the state of Ohio
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- One (1) year of professional experience practicing as a Registered Nurse (RN) in home health or similar setting
