Estimated Salary Range
The estimated salary range for this position is between $60,000 and $90,000 per year, depending on experience and qualifications.
Job Summary
As a Home Health RN, you will be responsible for providing coordinated skilled nursing care to patients of all age groups in their homes. You will work collaboratively with the interdisciplinary team to establish and achieve patient goals, ensuring high-quality patient care and excellent outcomes.
Key Responsibilities
- Assess home care patients identifying physical, psychosocial, and environmental needs.
- Complete OASIS assessments and visit paperwork according to agency policy.
- Communicate significant findings, problems, and changes to Clinical Manager and physician, and document all findings, communications, and appropriate interventions.
- Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided.
Required Qualifications
- Basic Cardiac Life Support certification within 30 days of employment start date.
- Registered Nurse license and associate degree or registered nurse diploma.
- Valid driver's license.
