Catholic Health Initiatives is seeking a skilled Continuing Care Coordinator RN to join our team. As a key member of our healthcare team, you will play a vital role in providing care management across the healthcare continuum for our members.
Responsibilities- Collaborate with physicians, staff, and other healthcare professionals to provide care management and support to our members.
- Work with our Population Health Care Management team to develop and implement system-wide quality improvement initiatives.
- Coordinate self-management support and disease registry activities for our members, working closely with physicians and staff to address specific quality improvement initiatives.
- Bachelor of Science in Nursing required; Master's of Science in Nursing preferred.
- Current unrestricted license by the State of Tennessee as a registered nurse.
- Five years of clinical and case management/health coach experience required.
- Clinic/Physician office home care public health and/or social service experience preferred.
