MDS Assessment Coordinator RN

Woodland Manor is seeking a skilled professional to fill the role of MDS Assessment Coordinator RN. The ideal candidate will possess strong assessment and communication skills, with the ability to work effectively with interdisciplinary teams. Key responsibilities include:
* Routinely assessing residents according to pre-set schedules or as needed for condition changes, hospital return, etc.
* Ensuring accurate observation, assessment, and communication of condition changes to appropriate personnel
* Coordinating the completion of assessments and Care Area Assessments (CAAs) by other departments, such as Social Services, Activities, Dietary, and obtaining corresponding signatures
* Completing other sections of the Minimum Data Set (MDS), quarterlies, CAAs, and care plans in the absence of other personnel
* Initiating care plans based on resident needs identified in the Resident Assessment Protocols and updating care plans according to guidelines
* Preparing and distributing the MDS schedule to the interdisciplinary team
* Transmitting the MDS to the state weekly (or as required) and maintaining accurate records, including a transmittal log
This role requires strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities. The successful candidate will be a team player with excellent communication and interpersonal skills.
