Job Summary

We are seeking a highly skilled and compassionate Physical Therapist to join our team. As a Physical Therapist at JAG, you will be responsible for assessing patients' strengths and weaknesses, developing creative and effective treatment plans, and delivering exceptional patient care.

Key Responsibilities
  • Collect and record patient care data, including initial evaluations, daily notes, reassessments, and discharge plans.
  • Develop and implement treatment plans based on evaluation results, goals, and best practices.
  • Collaborate with interdisciplinary teams, patients, and patient support systems to ensure comprehensive care.
  • Provide education and training to patients and their support systems on therapeutic activities, rehabilitation equipment, and community reintegration.
  • Oversee support staff, including PTA, ATC, and PT aides, to ensure seamless patient care.
Requirements
  • Bachelor's or Master's degree in Physical Therapy required; Doctorate degree preferred.
  • Licensed Physical Therapist in the state of practice.
  • Completed state-regulated CEU requirements.
  • Valid CPR license.
  • Experience with EMR software preferred.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
