**Job Description:** Mental Health Support Specialist

**Estimated Salary:** $55,000 - $65,000 per year

We are currently seeking a dedicated Mental Health Support Specialist to join our team at CDPC. The successful candidate will play a vital role in supporting clients with mental health needs, working closely with healthcare professionals to ensure comprehensive care.

Responsibilities:

  • Provide emotional and administrative support to clients in individual or group settings.
  • Assess clients' situations, identifying areas requiring support and intervention.
  • Develop and implement personalized treatment plans, working collaboratively with multidisciplinary teams.
  • Communicate client updates to relevant parties, ensuring seamless care coordination.
  • Facilitate referrals to external services and programs, ensuring timely access to care.
  • Maintain accurate and up-to-date client records, adhering to confidentiality protocols.
