Job Summary

We are seeking a highly skilled and experienced RN to fill the role of Patient Care Manager / Assistant Director of Nursing - Hospice. This is a dynamic leadership position that requires a strong background in hospice care and management.

Key Responsibilities
  • Staff Supervision and Development: Supervise, orient, mentor, and evaluate patient care staff to ensure they are equipped to deliver high-quality care.
  • Compliance and Accreditation: Ensure compliance with agency policies, state and federal laws, regulations, and accreditation standards.
  • On-Call Rotation: Participate in the manager on-call rotation to provide support and guidance to staff.
  • Talent Acquisition: Assist with talent acquisition to maintain adequate staffing levels.
  • IDT Meetings: Coordinate and facilitate Interdisciplinary Team (IDT) meetings to ensure effective communication and collaboration.
  • Quality Assurance and Performance Improvement: Assist with quality assurance and performance improvement efforts to drive continuous improvement.
  • Patient Care: Assist with patient case load as needed and manage the IDT process for multiple physicians and teams.
Requirements
  • Management Experience: 1 year of management experience required.
  • Hospice Care Experience: 2 years of hospice care experience required.
  • Nursing Education: Associates Degree in Nursing; Bachelors preferred.
  • Licensure: Valid RN Texas License.
  • Certifications: CHPN preferred.
  • Driver's License: Valid Texas Driver's License and Proof of Auto Insurance.
