Key Responsibilities- Supervise and guide field staff to ensure quality of care and adherence to Medicare requirements
- Conduct supervisory visits to observe home interactions and ensure quality indicators are met
- Instruct and observe HHA supervisions to ensure competency
- Develop and update nursing care plans
- Assist in the development of critical thinking skills and problem-solving
- Support clinicians in documentation excellence, COP compliance, case management, time management, and scheduling
- Conduct chart audits to ensure continuous survey-ready documentation
- Collaborate with MCPs on learning opportunities and support to staff
- Participate in performance improvement projects and provide data for QAPI reports
- Assess patient/family satisfaction during supervisory visits
- Associate's Degree in Nursing or Diploma Certificate in Nursing
- Minimum of three (3) years of clinical experience in hospice and/or home health
- NJ State Professional Registered Nurse License
- AHA Basic Health Care Life Support HCP Certification
- Certificate for OASIS Specialist - Clinical, Certified OASIS Quality Specialist, or Home Care Clinical Specialist Oasis within 1 year of entering position
