Position Overview

As a Registered Nurse Complex Case Manager, you will be essential to our healthcare team by:
  • Delivering specialized case management services for patients with complex medical conditions.
  • Engaging with individuals facing significant health issues, socioeconomic barriers, and mental health challenges.
  • Supporting patients in attaining optimal health and independence through tailored care strategies.
Key Responsibilities

1. Evaluate the physical, functional, social, psychological, environmental, learning, and financial needs of patients.

2. Identify challenges, objectives, and interventions aimed at fulfilling the patient's needs, prioritizing goals that reflect the preferences and desired involvement of the patient and caregivers.

3. Assist in developing an individualized care plan that includes objectives, goals, and actions tailored to the patient's requirements.

4. Implement appropriate interventions that demonstrate awareness of and respect for cultural diversity and the varied backgrounds of the population served. Utilize interpreter services as necessary.

5. Assess the patient's formal and informal support systems, including caregiver involvement and available community resources.

6. Execute and monitor the care plan to ensure the appropriateness and effectiveness of services, maintaining ongoing communication with relevant nursing staff.

7. Evaluate the patient's progress towards achieving goals, identifying barriers to compliance, and reassessing as necessary.

8. Research alternative treatment options and facilitate referrals to suitable providers.

9. Communicate with attending and primary care physicians and members of the care team regarding the patient's status.

10. Utilize motivational interviewing techniques to enhance patient engagement in their care plan.

11. Provide education, guidance, and support related to patient care objectives.

12. Advocate for patients and assist in resolving any barriers to care or adherence to the care plan.

13. Coordinate care and develop comprehensive treatment plans.

14. Refer patients to appropriate community resources and facilitate access to services.

15. Maintain accurate patient records and uphold patient confidentiality.

16. Measure outcomes and effectiveness of case management, including clinical, financial, quality of life, and patient satisfaction metrics.

17. Engage in professional development to stay current with case management practices and patient engagement strategies.

18. Promote disease prevention and health enhancement with patients and their families.

19. Assess the psychosocial and complex medical needs of all patients.

20. Address operational and clinical issues that may impact patient outcomes.

21. Attend required training sessions and staff meetings as assigned.

22. Participate in case reviews involving targeted patients.

23. Identify risk factors and educate patients on response strategies.

24. Encourage patient and family responsibility and self-management.

25. Document all relevant information in accordance with departmental policies.

26. Maintain knowledge of operational procedures and components of the case management program.

27. Advocate for chronic disease management concepts, health screenings, and preventive health initiatives for targeted patients.

28. Contribute to performance improvement projects and program development.

29. Assist in the collection, analysis, and benchmarking of utilization data.

30. Collaborate in the creation of protocols and guidelines for patient care management.

32. Perform other duties as assigned.

Qualifications

Bachelor's degree in nursing or a related field

OR

ADN with 5 years of case management experience and completion of BSN within 5 years of hire required.
