About the Role:

  • Under the supervision of an RN, you will provide direct nursing care to patients in their homes, as directed by physicians and other healthcare providers.
  • You will be responsible for implementing and evaluating patient care plans, providing focused systems and wound assessment, and documenting patient care in health records.
  • This role requires effective communication skills, particularly when working with individuals diagnosed with mental and behavioral disorders.

Requirements:

  • A graduate of an accredited State Board of Nursing LPN Program.
  • Registered as a LPN with the state of Oregon.
  • Current Basic Life Support (BLS) certification issued by the American Heart Association.
  • A valid Drivers License and proof of Insurance.
