**Job Summary:**

The Registered Nurse provides comprehensive nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The ideal candidate will have strong communication skills, be able to work independently, and possess a passion for delivering exceptional patient care.

**Responsibilities:**

* Conduct initial and comprehensive nursing evaluations to determine primary focus of care and develop individualized plans
* Collaborate with physicians, agency personnel, patients, and families to achieve continuity of care
* Ensure patients meet home health eligibility and medical necessity guidelines
* Maintain accurate, complete, and timely documentation in collaboration with physician orders

**Required Skills and Qualifications:**

* Current RN licensure in state of practice
* Current CPR certification
* Current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation
