Job Summary

This Nurse Case Manager role provides expert nursing care to patients while coordinating their discharge plans.

The ideal candidate will have a strong background in nursing and case management with the ability to work collaboratively with multidisciplinary teams.

Responsibilities

  • Provide high-quality nursing care to patients with complex medical needs
  • Coordinate discharge planning to ensure timely and safe transitions from acute care to post-acute settings
  • Collaborate with interdisciplinary teams to develop comprehensive care plans
  • Monitor patient progress and adjust care plans as needed
  • Communicate effectively with patients, families, and healthcare providers

Requirements

  • RN licensure in the state of Pennsylvania
  • Bachelor's degree in Nursing or related field
  • Minimum of 2 years of experience in case management or similar role
  • Strong critical thinking and problem-solving skills
  • Excellent communication and interpersonal skills
