Clinical Social Worker II

About the Role:
We are seeking a compassionate and experienced Clinical Social Worker II to join our team at AccentCare, Inc. as a Hospice Care Specialist. In this role, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Key Responsibilities:
- Provide emotional and practical support to patients and their families, addressing their unique needs and concerns.
- Collaborate with our interdisciplinary team to develop and implement comprehensive care plans that prioritize patient-centered care.
- Conduct thorough assessments and evaluations to identify patients' social, emotional, and environmental needs.
- Develop and implement strategies to address patients' social determinants of health, including housing, employment, and education.
- Provide counseling and therapy services to patients and their families, addressing issues related to grief, loss, and adjustment to illness.
- Facilitate communication and coordination between patients, families, and healthcare providers to ensure seamless care transitions.
- Participate in ongoing education and training to stay current on best practices in hospice care and social work.
Requirements:
- Master's degree in Social Work (MSW) or related field.
- Current licensure as a Clinical Social Worker (LCSW) in the state of operation.
- At least 2 years of experience in hospice care or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
