Job Title: Mobile MDS Coordinator

The Saber Community Support and Development Office is seeking an experienced Mobile MDS Coordinator to join our team. As a key member of our healthcare staff, you will be responsible for traveling to various locations to complete the Resident Assessment Instrument (RAI) process in accordance with state and federal laws, as well as our community's policies and goals. This role requires strong organizational and communication skills, as well as the ability to work effectively in a fast-paced environment.

Key Responsibilities:
  • Travel to multiple locations to complete RAI processes
  • Collaborate with healthcare teams to ensure accurate and timely assessments
  • Develop and implement care plans that meet the unique needs of each resident


Requirements:
  • Active, unencumbered Registered Nurse license in the state of practice
  • Two years of experience working in the RAI/MDS process
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
