Job SummaryWe are seeking a skilled and compassionate Licensed Practical Nurse II to join our team in a Skilled Nursing setting. As an LPN II, you will play a vital role in providing exceptional patient care and contributing to the success of our organization.
Responsibilities- Provide high-quality patient care in a Skilled Nursing setting, adhering to our organization's standards and guidelines.
- Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
- Administer medications and treatments as prescribed by physicians, and monitor patient responses to ensure optimal outcomes.
- Conduct thorough assessments of patients' physical and emotional needs, and document findings in a timely and accurate manner.
- Participate in ongoing education and training to stay current with best practices and advancements in the field of Skilled Nursing.
- High School Diploma or equivalent required.
- Two years of recent LPN experience preferred.
- Licensed Practical Nurse (LPN) licensure required.
- Basic Life Support (BLS) certification required.
- State IV Certification upon hire or within three months of hire/transfer required.
This is a full-time position working nights in a Skilled Nursing setting. The physical demands of this role require the ability to exert up to 50 pounds of force occasionally, and/or up to 20 pounds frequently, and/or up to 10 pounds constantly.
