Connected Health Care is seeking a skilled Physical Therapy Assistant to join our team in an outpatient setting. As a Physical Therapy Assistant, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.
Responsibilities
- Assist licensed Physical Therapists in the delivery of patient care, including exercises, modalities, and other interventions.
- Conduct patient assessments, develop treatment plans, and implement interventions to achieve patient goals.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in patient education and discharge planning to promote optimal outcomes.
- Collaborate with the healthcare team to identify and address patient needs, and to develop and implement strategies to improve patient care.
Requirements
- Current licensure as a Physical Therapy Assistant in the state of practice.
- Experience working in an outpatient setting, preferably with a focus on physical therapy.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
- Ability to lift, push, and pull equipment and supplies, as needed.
- Flexibility to work a variety of shifts, including days, evenings, and weekends.
