About the Role

We are seeking a highly skilled and motivated Clinical Pharmacist to join our team at PharMerica. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and ensuring the safe and effective use of medications in our long-term care facilities.

Key Responsibilities
  • Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
  • Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications.
  • Collaborate with the healthcare team to provide clinical and technical support, ensuring the safe procurement, storage, distribution, and use of medications.
  • Document all communication with physicians, nurses, and other healthcare team members in the medical record.
  • Implement policies and procedures for the safe handling and disposal of medications, as well as maintain required documentation in accordance with federal and state regulatory agencies.
Requirements
  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • One year experience in long-term care.
  • Licenses/Certifications: As required by government regulations and business needs.
  • Detail-oriented, goal-oriented, team player, and excellent oral communication skills.
