Job Description

We are seeking a skilled Occupational Therapist to join our team at FeldCare Connects. As a Home Health Specialist, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and providing interventions to improve patient wellbeing.

Key responsibilities include:

  • Providing services defined by state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
  • Leading patients in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
  • Observing, recording, and reporting patient response to treatment and changes to patient condition.
  • Instructing patients, families, caregivers, and the health care team in areas of therapy in which they can participate in assisting the patient.

Qualifications:

  • Ability and enthusiasm in working with all ages.
  • Occupational Therapist license and registration by the state.
  • Completion of an accredited Occupational Therapist program.
  • Bilingual a plus.
