Patient-Centered Care
This role involves direct care to patients as part of our chronic disease management program, including medication prescription and tailored care plans. You will partner with Administrative Leaders to develop and enhance program initiatives through effective dyad leadership. Your expertise will drive program improvement based on key metrics identified in Performance Barometers.
Key Responsibilities
- Lead the clinical aspects of the Driver Wellbeing Pilot Program.
- Oversight of chronic disease management, wellbeing assessments, and clinical wellness & testing programs.
- Direct patient care, including medication prescription and care plan creation.
