Caregiver - Hospice Support Specialist

Job Summary: We are seeking a compassionate and skilled Home Health Aide to join our team at Horizon Home Care & Hospice. As a Hospice Care Assistant, you will provide direct assistance with activities of personal care, maintenance of patient health, and facilitation of treatment for patients in their homes.
Key Responsibilities:
- Provide assistance with daily living activities, such as bathing, dressing, transfers, and toileting, under the supervision of a Registered Nurse or Therapist.
- Review care plans with patients and families to ensure understanding and compliance.
- Notify Case Managers of any issues or concerns regarding care plans and provide updates on patient progress.
- Work cooperatively with patients, care providers, and family members to ensure a smooth and effective care experience.
- Complete mobile care tasks, including mileage logs, in accordance with Agency policy.
Requirements:
- High school diploma or equivalent required.
- Previous experience in home health care or a related field preferred.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
