Role Summary

The Clinical Care Coordinator is responsible for fostering a proactive, enduring relationship with patients to enhance the management of their health needs. This role involves assessing patient requirements, planning, coordinating, and evaluating healthcare services, with the aim of empowering patients and their families to access necessary resources and adopt healthier lifestyles, thereby ensuring they remain in the most suitable care environment. The focus areas include: 1) population health management, 2) patient self-management assistance, 3) care transitions, 4) resource linkage, and 5) effective resource utilization. Collaboration and active communication with a multidisciplinary team, including healthcare providers, pharmacists, social workers, behavioral health experts, and nursing staff, are essential for optimal patient management.

Key Responsibilities

  • Employs the RN process to guide the healthcare team's activities towards achieving optimal patient outcomes, resource efficiency, and clinical excellence.
  • Engages with patients, healthcare professionals, and the community to ensure continuity of care and coordination of services, documenting care plans across various settings.
  • Conducts comprehensive assessments for identified patients, ensuring knowledge of appropriate care services to address their needs, disease management, and clinical expectations.
  • Maintains precise case records for each patient referred.
  • Ensures thorough and accurate documentation, utilizing appropriate databases and record-keeping systems.
  • Stays informed about reimbursement criteria and documentation requirements for various insurance programs, including Medicaid and Medicare.
  • Demonstrates leadership in nursing practice by evaluating personal performance against professional standards and regulations. Physical Demands Work involves walking, standing, lifting, reaching, bending, and stooping. Must be able to lift at least thirty-five pounds to shoulder height. Travel between locations may be required. Frequent verbal and written communication in English is necessary. Must possess intact vision and hearing, finger dexterity, critical thinking skills, and the ability to focus. Must be capable of responding swiftly to changes in assignments. Occasional exposure to noise and environmental conditions such as dust and chemicals may occur. Qualifications A Bachelor of Science in Nursing (BSN) is required; a Master's degree is preferred. A current RN license or temporary license as a Registered Nurse Petitioner in the relevant state is mandatory. A minimum of two years of experience in healthcare is required, with experience in case management or discharge planning in settings such as acute care, home care, or managed care. Professional certification must be obtained within three years of hire. Clinical competence in disease management and case management principles is essential. Strong interpersonal communication, negotiation, problem-solving, organizational, and time management skills are necessary, along with the ability to work independently and collaboratively. BLS certification is required per policy guidelines.
