About the Role:

We are seeking a skilled Clinical Coordinator to join our team at DaVita. As a Clinical Coordinator, you will play a vital role in ensuring the highest level of care for our patients with end-stage renal disease (ESRD) or chronic kidney disease.

Key Responsibilities:

  • Coordinate patient care and services, including tracking vital signs, lab values, and outcome management.
  • Ensure the safety, comfort, and well-being of patients by delivering exceptional care.
  • Collaborate with the clinical team to achieve quality improvement initiatives and clinical goals.

Requirements:

  • At least 6 months of dialysis experience.
  • Current Registered Nurse (RN) license in the state of practice.
  • CPR certification required.
  • At least 18 months of registered nursing experience.
  • Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA required.
