Position Overview
The PCMH Physician is responsible for delivering direct medical care to all patients within their assigned panel. This role may also involve supervising medical trainees who provide care to patients, ensuring high standards of clinical education and practice. Additionally, the physician will oversee the management of clinical panels in collaboration with the entire PCMH team, particularly working alongside the Nurse Care Manager to support high-risk patients.
Key Responsibilities
- Deliver direct patient care and oversee the clinical activities of trainees.
- Document all patient interactions in the electronic health record promptly.
- Review diagnostic test results and ensure effective communication of results to patients.
- Respond to messages and notifications from staff within a timely manner.
- Facilitate multidisciplinary conferences for complex patient cases as necessary.
- Utilize health maintenance tools to address care gaps effectively.
- Communicate care priorities and follow-up plans to team members, especially for high-risk patients.
Qualifications
- MD, DO, or equivalent medical degree required.
- Board certification or eligibility in Family Medicine, Internal Medicine, or Internal Medicine/Pediatrics.
- Possession or eligibility for a California medical license or equivalent certification.
