Statewide Travel Palliative Care Nurse Practitioner

Job SummaryWe are seeking a Statewide Travel Nurse Practitioner to join our Advanced Illness Management team. This role will involve facilitating and providing advanced illness management and palliative care services to patients across the state. The successful candidate will be responsible for providing individualized treatment plans and comprehensive care to patients in their homes, under the direction of the Palliative Medical Director or Primary Care physician.
Key Responsibilities- Provide individualized treatment plans and comprehensive care to patients in their homes
- Travel extensively throughout the state to provide patient care and other essential responsibilities
- May be required to travel regionally on occasion
- Flex or alternative schedules available
- Master of Science in Nursing from an accredited school of nursing or equivalent
- 2+ years of Nurse Practitioner experience
- Current license as a registered nurse and equivalent as required by state laws for Nurse Practitioner licensure
- Active and unrestricted Nurse Practitioner certification/license
- Multi-state licensure desirable for Travel Nurse Practitioner role or willingness to obtain additional state licensure
- Practice experience with Part B documentation and coding
- Collaborative agreement with a supervising physician or Palliative Medical Director
- Part B Provider certification
- Federal DEA license
- State DEA license or state-specific prescribing authority (if required)
- ACHPN certification in Hospice and Palliative Care (preferred)
- Nurse Practitioner specialty in Family or Adult Medicine, Acute Care (preferred)
