Key Responsibilities:

  • Participate in ongoing interdisciplinary assessment of the patient
  • Complete initial and ongoing comprehensive assessments of the patient's needs
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s)
  • Provide services that are ordered by the physician as indicated in the plan of care
  • Counsel and educate the patient and family in meeting healthcare-related needs
  • Initiate appropriate preventative and rehabilitative nursing procedures
  • Refer to other services as needed
  • Monitor and evaluate outcomes of patient care
  • Communicate with appropriate staff regarding changes in the patient's needs
  • Supervise paraprofessional staff
  • Effectively communicate with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care
  • Communicate with all physicians involved in the plan of care and other healthcare practitioners related to the current plan of care
  • Participate in Quality Assessment and Performance Improvement (QAPI) program
  • Participate in in-services or training, as requested

Requirements:

  • Graduate of an accredited school of nursing and current RN License in SC or compact license
  • 2 years' experience as a nurse with 1 year of experience in home health
  • Proficient computer skills and workplace software knowledge
  • Ability to work with people, function well under stress, use clinical judgment, be creative, and accept challenges
  • Able to effectively deal with multiple changes based on patient needs and scheduling
  • Clinical proficiency in nursing, strong communication skills, and compassion
  • CPR Certification
  • Valid driver's license and proof of insurance
