Job Summary

This position is an excellent opportunity to make a meaningful impact in the lives of our members. As a Social Work Case Manager, you will play a critical role in coordinating care and services to help our members achieve their optimal level of independence and quality of life.

Key Responsibilities
  • Collaborate with the healthcare team to identify and address the social determinants of health that impact our members' well-being.
  • Conduct comprehensive psychosocial assessments to develop and implement personalized plans of care.
  • Facilitate access to community resources and services to support our members' needs.
  • Provide education and support to members, families, and caregivers to promote self-awareness and knowledge of current case management standards.
  • Stay up-to-date on laws, regulations, and policies related to the practice of social work in the local market.
Requirements
  • Master's degree in social work (MSW) required.
  • Minimum three (3) years of clinical experience plus two (2) years in case management required.
  • Minimum one (1) year of experience with the defined population required.
  • Licensed Clinical Social Worker (LCSW) certification required within six months of hire.
Preferred Qualifications
  • Experience with computer software programs in a Windows environment preferred.
  • Knowledge of community systems and resources in the defined service area preferred.
  • Knowledge of regulatory issues for the Mid-Atlantic area preferred.
