Social Work Case Manager Level I

Kaiser Permanente is seeking a highly skilled Social Work Case Manager Level I to join our team in Washington D.C.
Job SummaryThis is a challenging and rewarding role that requires a strong understanding of social work principles and practices. As a Social Work Case Manager Level I, you will be responsible for collaborating with members of the healthcare team to facilitate the coordination of appropriate, cost-effective services that are consistent with members' plans of care.
Key Responsibilities- Conduct comprehensive psychosocial assessments to evaluate patient goals, social support systems, resources, health status, functional limitations, psychological status, environmental factors, and response to treatment.
- Develop and monitor a plan of care designed to promote the member's optimal level of functioning and enhance the quality of life.
- Identify, facilitate, and advocate for appropriate organizational and community resources to meet the plan of care and ensure they are implemented in a cost-effective, efficient, and timely manner.
- Ensure consistent and reliable documentation of case management activities in compliance with all organization and department standards.
- Analyze patient and program outcomes to identify improvements in program, quality, and cost-effectiveness of case management activities.
- Bachelor's degree in social work required.
- Licensed Bachelor Social Worker (Maryland) within 6 months of hire.
- Licensed Social Work Associate (District of Columbia) within 6 months of hire.
- Licensed Baccalaureate Social Worker (Virginia) within 6 months of hire.
