About the Role

We are seeking a highly skilled Healthcare Coordinator to join our team at Bridgewell. This role is ideal for an experienced Registered Nurse looking to make a meaningful impact in the lives of individuals with disabilities.

Job Summary

The successful candidate will be responsible for coordinating medical nursing services, overseeing healthcare services, and providing supervision to assigned residential nursing staff. With a strong focus on patient-centered care, you will work closely with our multidisciplinary team to ensure the best possible outcomes for our clients.

This is a full-time position working Monday through Friday, 8:00 AM to 4:00 PM (40 hours). The salary for this role is $65,000-$80,000 per year, depending on experience.

Key Responsibilities
  • Provide supervision of daily nursing activities within assigned geography
  • Assess individual needs and develop appropriate plans of care with nursing staff
  • Oversee daily clinical needs by collaborating with nursing team members, physicians, visiting nurse associations, and program staff
  • Support nursing team members to ensure competence with DPH MAP system, clinical training, and medical issues
Requirements
  • Associate's or bachelor's degree in nursing and current RN license in Massachusetts
  • At least one year of experience working with individuals with disabilities
  • One year of experience in a community setting
