We are seeking a highly skilled and experienced Inpatient Care Management Director to join our team at Ocean State Job Lot. As a key member of our Care Management Department, you will be responsible for overseeing the daily operations of our Inpatient Care Management team, ensuring that patients receive high-quality care and services.
Key Responsibilities- Supervise and direct the Inpatient Care Management team, including RN Care Managers, Lead CM RN, CM Coordinators, Discharge Planners, and UM CM Staff.
- Assess patients for level of care, appropriateness for admission or observation, HLOC transfers, discharge assistance, and follow-up care in collaboration with the ED and SW care management team.
- Develop and implement Care Management policies and procedures to ensure compliance with regulatory requirements and organizational standards.
- Provide clinical supervision for scheduling, UKG Pro payroll, coaching/corrective action, and educational needs to ensure that Care Management policies and processes are carried out for the organization.
- Oversee the electronic referral system, documentation of CM through Cerner Healthcare EMR, and work with Clinical Informatics (CIT) to develop or improve CM documentation and handoff processes.
- Collaborate with other managers within the Population Health Program to support the entire Care Management Department.
- Bachelor's degree in nursing required; Master's degree in nursing or related area preferred.
- Five (5) years acute care hospital, health plan, and/or ancillary services required.
- Prior Supervisory or Management Experience in a clinical role required.
- RN license required; CCM or ACM certification required within 2 years of hire.
- Basic Life Support (BLS) certification.
