Job Title: Registered Nurse - Hospice

Alternate Solutions Health Network LLC is seeking a compassionate and skilled Registered Nurse (RN) to join our team in providing high-quality care to patients in need of hospice services.

About the Role:
  • As a Registered Nurse - Hospice, you will be responsible for providing end-of-life care to patients and their families, working closely with an interdisciplinary team to develop and implement individualized care plans.
  • You will utilize your clinical skills to assess patient needs, develop and implement care plans, and provide education and support to patients and their families.
  • Our ideal candidate will have a strong commitment to patient-centered care, excellent communication skills, and the ability to work autonomously in a fast-paced environment.
Responsibilities:
  • Plan and implement individualized care plans for patients, taking into account their unique needs and goals.
  • Assess patient needs and develop strategies to address pain, symptom management, and other clinical concerns.
  • Provide education and support to patients and their families, promoting self-care and empowerment.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Document patient progress and outcomes, maintaining accurate and timely records.
Requirements:
  • Current RN licensure in the state of employment.
  • Minimum one year of experience as an RN in an acute care setting, with a preference for hospice or palliative care experience.
  • Valid driver's license and auto insurance.
  • Ability to work autonomously and as part of a team, with excellent communication and interpersonal skills.
