- Coordinate Care Services: Coordinate and manage all aspects of nursing care and services provided to patients and families, ensuring timely, cost-effective, and respectful delivery of care.
- Lead Interdisciplinary Teams: Lead and collaborate with interdisciplinary teams to promote effective communication, coordination, and delivery of patient care services.
- Advocate for Patients: Advocate for patients and families, ensuring their needs and concerns are addressed and respected.
- Participate in Quality Improvement: Participate in quality improvement initiatives to identify and address opportunities for improving patient care and services.
- Education: Bachelor's degree in Nursing (BSN) or Associate's degree in Nursing (ASN) with 2 years of experience.
- Licensure: Current Registered Nurse licensure in Tennessee.
