About the Role

We are seeking a skilled Speech Therapist to join our team in providing high-quality care to patients in their homes. As a Speech Therapist PRN, you will be responsible for assessing and evaluating patient care needs related to treating speech and language disorders, and functional training in communication, swallowing, and cognitive impairments.

Key Responsibilities
  • Provide services within the scope of practice as defined by state laws governing the practice of speech therapy, in accordance with the plan of care, and in coordination with other members of the healthcare team.
  • Evaluate patient's level of function, status of all body systems as required for CMS documentation, and speech therapy needs using standardized and/or criterion-referenced tools. Consult with physician in development of therapy plan of care.
  • Treat patients' speech production, fluency, language, cognition, voice, resonance, swallowing, and auditory habilitation/rehabilitation using evidence-based standards of practice.
  • Observe, record, and report to nurse supervisor and/or physician patient's response to treatment and changes in patient's condition.
  • Instruct patient, family, and/or caregiver and other members of healthcare team in areas of therapy within scope of speech-language pathology.
  • Visit patients within entire service area of their respective agency and other nearby LHC agencies as needed.
  • Complete progress notes for each visit and submit progress notes per LHC policy.
  • Complete all patient evaluations and develop ST plan of care within state-specific guidelines. Report outcomes of evaluation, goals, and anticipated projected frequency of care to nurse supervisor within 24 hours of evaluation.
  • Participate in staff conferences and committees as requested by agency. Provide in-service education as requested by agency leadership. Prepare written discharge summary and/or written summary report on patient's condition at least every 60 days.
  • Adhere to LHC Therapy Standards of Practice in provision of services to all patients.
  • Responsible for accessing education to maintain and enhance clinical knowledge and skills that are up to date with current standards of care for profession and to maintain state-issued licensure.
  • Participate in weekend rotation to provide services for their respective agency and other nearby LHC agencies as needed.
Qualifications
  • Must be currently licensed in Speech Therapy in state of FL.
  • Current CPR certification.
  • Must have current driver's license and vehicle insurance, and access to dependable vehicle or public transportation.
