About the Role

We are seeking a dedicated and compassionate Licensed Vocational Nurse to join our team as a Certified Home Health and Hospice Nursing Specialist. In this role, you will provide high-quality nursing care and support to patients in their homes, working closely with our interdisciplinary team to ensure seamless coordination of care.

Responsibilities
  • Administer skilled nursing care to patients of all ages, following established care plans and protocols.
  • Liaise between field and office teams, ensuring effective communication and coordination of patient care.
  • Assist patients and families in learning self-care techniques and promoting independence.
  • Work collaboratively with the RN to carry out the plan of care and make decisions related to patient needs.
Requirements
  • Graduate of an accredited school of vocational nursing.
  • Current Texas LVN license and valid State Drivers License.
  • Two years of experience as a Licensed Vocational Nurse in a clinical care setting, preferably in hospice or home health.
  • Nursing skills as defined by generally accepted standards of practice, including wound care, infusion, vent, and trach experience (preferred).
  • Good interpersonal skills and ability to work effectively with patients, families, and healthcare teams.
  • Reliable transportation and valid auto liability insurance.
  • Spanish speaker preferred but not required.
Working Conditions

This role requires working in patients' homes in various conditions, with possible exposure to blood and bodily fluids and infectious diseases. You will also need to work a flexible schedule, travel locally, and be able to lift up to 50 lbs and move patients as needed.

Salary and Benefits

We offer a competitive hourly rate of $33-35 per hour, or $45 per visit depending on field or office preference and experience.
