We are seeking a highly skilled and compassionate Home Health Registered Nurse Case Manager to join our team at Signature Healthcare at Home. As a key member of our team, you will be responsible for providing high-quality patient care and case management services to patients in their homes.
Responsibilities- Conduct comprehensive assessments of patients and develop individualized care plans to meet their unique needs.
- Provide skilled nursing care and education to patients and their families, promoting independence and self-sufficiency.
- Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination and communication.
- Manage patient caseloads, prioritize tasks, and maintain accurate records and documentation.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current licensure as a Registered Nurse in the state of practice.
- 1-2 years of recent acute care experience in an institutional setting, with a preference for home health experience.
- Excellent observation, verbal, and written communication skills, with the ability to work independently and as part of a team.
- Ability to lift, position, and transfer patients, as well as lift supplies and equipment.
- Valid driver's license and reliable transportation.
