Job Summary:

The Certified Nurse Assistant (CNA) provides high-quality care to participants at our Adult Day Health Center. As a CNA, you will assist participants with activities of daily living, provide restorative and supportive care, and collaborate with multidisciplinary teams in care planning.

Responsibilities:

  • Assist participants with bathing, dressing, grooming, hair care, mouth care, eating, and toileting.
  • Document care provided in accordance with established policies, guidelines, and applicable regulations.
  • Carry out, report, and document care given in an effective, timely manner as observed by the RN and through periodic record reviews.
  • Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen, and provides other aides as assigned or needed.
  • Monitor participants' health status, including vital signs, weights, skin condition, appetite, and changes in functional ability, and report pertinent information to the clinical supervisor in a timely manner.
  • Collaborate with multidisciplinary teams in care planning for participants.
  • Monitor appetite and look for changes in functional ability and skin condition.
  • Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary.
  • Report any variance, accident, or unusual occurrence to the appropriate personnel.
  • Assist with loading and unloading of participants from vehicles.
  • Assist the transportation department with participants' take-home supplies.
  • Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments, supporting participants while in transit and accompanying participants in and out of facilities.
  • May rotate into In-Home Services as a Homecare CNA as deemed necessary by the Center Manager.
  • Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed.
  • Attend in-service and continuing education programs as scheduled and necessary.
  • Other duties as assigned.

Requirements:

  • Active and valid state-issued Certified Nursing Assistant (CNA) license.
  • Current CPR certification or the ability to obtain CPR certification.
  • High School Diploma or GED equivalent.

Preferred Qualifications:

  • Experience in a care setting for frail and elderly patients participating in a group environment.
  • Bi-lingual.
