Stroke Program Navigator

We are seeking a highly skilled and experienced Stroke Program Navigator to join our team at Reid Health. As a key member of our stroke team, you will be responsible for overseeing clinical quality and monitoring evidence-based standards of care, coordination of the stroke team, ensuring regulatory compliance, ongoing monitoring of telestroke performance, patient satisfaction, and development of educational materials.
Key Responsibilities- Monitor the stroke care of patients presenting in the Emergency Department from admission to stabilization to discharge or transfer to a highly level of care, if required.
- Responsible for and actively participates in the education and training of medical and clinical staff involved in stroke care.
- Actively participates in correcting deficiencies in the stroke care and coordinating performance improvement processes through multidisciplinary teams.
- Organizes and attends meetings related to stroke care in the organization.
- Responsible for data abstraction and analysis, reporting back to the Stroke Medical Director and the stroke team.
- Responsible for education of staff as well as community EMS and any public outreach events.
- Coordinates regulatory compliance as it relates to the Stroke Program.
- Annual review of Policies and Procedures as well as Protocols relating to the Stroke Program.
- Monitoring of the telestroke service as well as outcomes and any opportunities for process improvements.
- Provide support of inpatient stroke population as well as in follow-up in the outpatient clinic.
- Assure core quality measures are known and being met, providing feedback when these goals are not reached.
- Provider chart review and process improvement on all Activase cases.
- Provide organizational feedback and communication to facilities transferring patients to this organization in an effort to improve processes and relationships.
- BSN from an accredited nursing program.
- Experience in medical office or health care setting required.
- Experience in neuroscience care with an emphasis on stroke care and neurology preferred.
- Leadership experience preferred.
