Job Summary

Professional Care Medical Practice, P.C. is seeking a highly skilled and dedicated Behavioral Health Professional to join our team in the medical department at the Monroe County Jail in Rochester, NY.

Key Responsibilities
  • Conduct comprehensive clinical assessments and develop treatment plans for inmates/patients.
  • Monitor and coordinate mental health services, including risk assessments and referrals to psychiatry when necessary.
  • Serve as the mental health liaison between the medical unit and facility staff.
  • Develop and conduct in-service education programs for staff.
  • Collect and analyze data to prepare monthly reports on performance.
Requirements
  • Licensed as a LMHC, LMSW, or LCSW in the state of New York.
  • Master's degree or greater in Social Work, Psychology, or related Human Service field with a concentration in clinical course work, training, and/or experience.
  • Minimum of two years of experience and familiarity with substance abuse/MAT counseling preferred.
  • Valid driver's license and reliable transportation.
