Hospice LPN Job Description

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at a progressive and well-run Hospice organization in the Savannah, GA area.

About the Role

The LPN will be responsible for providing high-quality patient care and support to individuals and their families in a Hospice setting. This is a rewarding opportunity to make a meaningful difference in the lives of others.

Responsibilities
  • Provide direct patient care and support to individuals and their families in a Hospice setting.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
  • Administer medications and treatments as prescribed by the physician.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Licensed Practical Nurse (LPN) in the State of Georgia.
  • 1 year of Hospice or Palliative Care experience preferred.
